Dental and Vision Insurance for Freelancers: What You Need to Know

Health insurance for freelancers gets all the attention, but dental and vision coverage are equally important and often overlooked. Unlike medical insurance, dental and vision plans are not included in standard ACA Marketplace plans and must be purchased separately. Understanding your options and whether standalone coverage makes financial sense is key.

Are Dental and Vision Covered by ACA Plans?

Adult dental and vision coverage is not an essential health benefit under the ACA, meaning most standard Marketplace health plans do not include it. Pediatric dental and vision coverage is required for plans covering children. If you want dental and vision coverage as an adult, you need to purchase standalone plans.

Standalone Dental Insurance Options

Traditional Dental Insurance

Traditional dental insurance works like health insurance with premiums, deductibles, and annual maximums. Most plans cover preventive care (cleanings, X-rays) at 100 percent, basic procedures (fillings) at 80 percent, and major procedures (crowns, root canals) at 50 percent. Annual maximums are typically $1,000 to $2,000.

Monthly premiums for individual dental coverage typically range from $20 to $50 for basic plans and $40 to $80 for more comprehensive coverage. Family dental plans run $60 to $150 per month.

Dental Discount Plans

Not actually insurance, dental discount plans charge a membership fee ($100 to $200 per year) for access to a network of dentists who agree to provide services at reduced rates — typically 10 to 60 percent below standard fees. These can be cost-effective for people who need significant dental work and cannot afford traditional insurance premiums.

Buying Dental Through the ACA Marketplace

You can purchase standalone dental plans through the ACA Marketplace alongside your health plan. These are called Qualified Dental Plans (QDPs) and come in two tiers: low and high. Buying dental through the Marketplace does not affect your health insurance subsidy eligibility.

Standalone Vision Insurance Options

Vision plans are relatively inexpensive ($15 to $30 per month) and typically cover one annual eye exam and an allowance toward glasses or contact lenses ($150 to $200 allowance is common). For most freelancers who wear glasses or contacts, standalone vision insurance pays for itself within the first claim.

Popular standalone vision plan providers include VSP, EyeMed, and Humana Vision. You can purchase these plans independently without needing to buy health insurance through the same company.

The Cost-Benefit Analysis

For dental insurance, the break-even calculation is straightforward. If your annual premium is $480 ($40 per month) and your plan covers two cleanings and X-rays at 100 percent — which typically costs $300 to $500 at list price — you are already near break-even on preventive care alone. Any fillings, crowns, or other work in the year tips the math firmly in favor of having coverage.

For vision, if you wear glasses or contacts and need an annual exam plus new lenses or contacts, a $20 per month plan almost certainly saves you money compared to paying out of pocket.

The Tax Deduction Advantage

Both dental and vision insurance premiums are deductible under the self-employed health insurance deduction, along with your medical premiums. This reduces the effective monthly cost of both types of coverage by your marginal tax rate — typically 22 to 32 percent for most freelancers.

Recommended Approach for Most Freelancers

Purchase a standalone dental plan if you have not had significant dental work recently and want to maintain regular preventive care. Add vision coverage if you wear corrective lenses. Both types of coverage are inexpensive enough that the tax deduction and benefit value make them worthwhile for most freelancers.
